IMPORTANT NOTES:

• All teams must complete in FULL and submit a Team Nomination Form prior to commencement of ACBA tournament. Failure to submit a form will result in no insurance coverage for any member of the team.
• Players listed in rows 11 and 12 are regarded as injury reserve players. These players alone are eligible to replace any of the first 10 players injured during the course of a tournament.
• After 6 weeks of the tournament, no changes may be made to a team's roster. Teams fielding players NOT on this Nomination Form will be deemed unregistered and penalities shall apply.

A deposit will need to be made for nomination
to be accepted. The following deposits are to
be made for the corresponding divisions:

• Division 2 -$450
• Division 3 -$400
• Division 4 -$400
